ARTIST INFORMATION FOR HEALTH DEPARTMENT

Every artist tattooing at the Grand Rapids Tattoo Festival must fill out this information. You will need to upload a Bloodborne Pathogens Certificate that is valid for October 24-26, 2025. If you need a new BBP Certificate, we recommend this one, which is OSHA certified.
FORMS ARE DUE NO LATER THAN 9/23/2025.

Work history should include approximate start and stop year for everywhere you have worked as a tattoo artist. This should also include start and end date for your apprenticeship.
