HEALTH DEPARTMENT FORMS
County and State health forms are charged per shop, not per artist. The more of your coworkers and friends you can pair into a singular “group/shop”, the lower your health department fees will be. These forms will determine which artists you are boothed next to.
You will be sent an invoice for your fees after completion of these forms. Please do not mail in payment to state or county.
